The date on Forms 940, 941 and 943 should be left blank. This form is RARELY signed by the preparer on the date it is produced.
Also, the date for the taxpayer should be left blank - to be signed and dated when the taxpayer reviews this form. It is NEVER signed on the date it is prepared if a paid preparer is doing the work.
